and Pathological investigations for patients
of Rampurhat health unit on contract
basis for a period of three (03) consecutive
years from the date of acceptance/offer of contract
as per terms and conditions given with the tender form.

: Rs.1, 000/- (Rupees one thousand only).

Tender
form can be obtained from the office of the Chief Health Director,
Orthopaedic Hospital, E. Rly., 222, Biplabi Haren Ghosh Sarani,
Orthopaedic Hospital, E. Rly. Howrah-711101 on any working
day excluding Saturday & Sunday or any Holiday from
16/06/2008
to 20/06/2008
between.11.00 hrs to 14:00 hrs. on production of original
money receipt obtained
from the chief Cashier E. Rly., Kolkata-I or Booking Supervisor,
E. Rly, Howrah by depositing Rs.1,000/- (Rupees one
thousand only) towards cost of tender form. Tender
form or cost of it is neither refundable nor transferable.
If the last date of selling tender form falls on Saturday,
Sunday, holiday or unavoidable circumstances like bandh the
same will be given on next working day within stipulated time
mentioned above.

Tender
document can be downloaded during the above mentioned period
from Eastern Railway’s website, ”http://www.easternrailway.gov.in
&
http://www.tendertimes.com“
and the same can be used as tender document for submitting
the offer. In such case the cost of tender form has
to be paid in the shape of demand draft / banker’s cheque
of any Nationalized Bank of India amounting to Rs.1,000/-
(Rupees one thousand only) drawn in favour of FA&CAO,
E.Rly / Kolkata and must be enclosed with the tender form.
In case the tender offer is not accompanied with the
valid demand draft / banker’s cheque for the
cost of tender document the tender offer will be summarily
rejected. The demand draft / banker’s cheque is not
refundable.

Sealed
tender must be dropped in the tender box kept in the office
of Chief Health Director, 222, Biplabi Haren Ghosh Sarani,
Orthopaedic Hospital, E. Rly. Howrah-711101 from
16/06/2008 to 20/06/2008
between 11.00 hrs. to 14.00 hrs. except Saturdays, Sundays,
and holidays or unavoidable circumstances. The tender box
will be closed on 20/06/2008
at 14.00 hrs.
and will be opened on 20/06/2008
at 15.00 hrs.

If
the office of the Chief Health Director, Howrah remain closed
for unforeseen reasons on the last date of dropping of tenders,
the tenders can dropped on the next working day and the tender
box will be closed and opened on the same day, the time however
remaining unchanged as mentioned above.

The
railway authority reserves the right to accept or reject any
tender offer without assigning any reason whatsoever.
|
| |
Special Attention
|
| 1) |
The
Contract will be for a period of 03 (three) years
from the date of award of offer / acceptance of contract.
|
| 2) |
Separate tender forms
to be used for Pathological and Radiological
investigations.
|
| 3) |
Rates
must be quoted separately for all Pathological and Radiological
investigations (Otherwise offer will be summarily rejected)
as per list enclosed in ANNEXURE-A (Pathological) and ANNEXURE-B
(Radiological).

| 4) |
All
Pathological and Radiological investigations have to be done
at Rampurhat and nearest to the health unit.
|
| 5) |
Terms
& conditions
and instructions are available with the tender form.
|
| 6) |
Credential
of similar type of work in Govt., Semi-Govt. organization
with satisfactory performance report may be submitted with
the tender form.
|
| 7) |
Performance guarantee & Security deposit
:
|
| |
Security
money amounting to 5% of the accepted tender value will be
recovered at the rate of 10% of the bill amount per month
till the full amount is recovered and a performance guarantee
amounting to 5% of the accepted tender value in the form of
an irrevocable bank guarantee will have to be furnished
to this office before signing the agreement and within fifteen
(15) days from the date of award of offer / acceptance of
contract and it should be valid for the entire period of contract
and two (02) months after the expiry of the contract period,
failing which the tender offer will be treated as cancelled
and no further correspondence on the subject will be entertained.
Performance Guarantee shall be released after satisfactory
completion of the work. No interest will be paid for the said
security money and performance guarantee.

Earnest
money to be given separately for Pathological and Radiological
investigations along with the tender forms.
|
| 9) |
Tender
form without earnest money will be summarily rejected.
